Before you start

This form organises information for one employee, one intended employer and one new employment. It does not submit anything to REINZA, HMRC or the employer.

Completion does not verify identity, bank details, National Insurance information, references, qualifications or right to work. It does not calculate payroll, tax, student-loan deductions or pension duties.

Have the information ready

You may need personal and contact details, P45 information, bank details, right-to-work information and employment history. Use Save Draft only if you need a local plaintext copy.

Limitations and official sources

This tool collects employee-supplied information. The employer remains responsible for payroll reporting, right-to-work checks, data protection, references, qualification checks and every other external process.
