Before you start
Have your personal and contact details, bank details, National Insurance information, any P45, right-to-work information and employment history available. Complete the form for one employer and one new employment. REINZA does not receive, store or verify what you enter.
Names, dates and contact details
Use your official name where the form asks for it. Enter real calendar dates and check telephone, email and address details carefully. Format checks do not prove that information is genuine.
PAYE, P45 and starter questions
Copy P45 figures exactly when a P45 is available. The leaving date may be in the future, for example during a notice period. The three starter questions are separate from the P45: answer them from what has actually happened since 6 April. If you are unsure, choose “Not sure” instead of guessing so the employer or payroll team can review it.
Student and postgraduate loans
Use your current loan information and the HMRC stopping conditions shown by the form. The form records your answer; it does not calculate deductions.
Bank details
Check the account holder name, sort code and account number before downloading the PDF or Excel file. The form does not contact the bank or use Confirmation of Payee.
Right to work
Choose the route that best describes the evidence you expect to provide. This only organises employee-supplied information. Your employer must complete the prescribed official check and decide which route applies.
Qualifications and documents
Record qualifications only where relevant and indicate whether requested documents are available or already provided. Do not upload documents: the form has no upload or transmission facility.
National Insurance number
Enter your National Insurance number only if you know it. The form checks its shape but does not verify it with HMRC. Do not guess or invent a number.
Drafts, review, PDF and Excel
The form can create three different files. Each has its own purpose and its own button, and each button creates one file only.
- Draft (JSON) is for you. Save Draft creates a plaintext file on your device so you can continue later with Load Draft, which reads the file locally and replaces current entries only after confirmation. It is not the record you give to your employer.
- PDF is the readable record. On the final step use Download PDF: the form creates an A4 file with page numbers in your browser. Use it for checking, keeping a copy and signing on your first day.
- Excel (.xlsx) is the structured data record. On the final step use Download Excel (.xlsx). It contains the same information as the PDF in a form your employer's payroll, accounting or HR team can process without re-typing it.
Download the PDF and the Excel file one after the other; the form never downloads two files from one click, so your browser does not need any special multiple-download permission. Give both files to your employer. On Review, each issue states why it needs attention and provides an Edit action; use the section menu to return to any reached section. An incomplete record is clearly marked in both files. All three files contain sensitive information in plain form, so store them securely and delete them when no longer needed.
Giving the form to your employer
Give your employer both the PDF and the Excel file, using only the secure return method your intended employer has given you. Do not send sensitive personal, bank or right-to-work information to an unverified address or through an insecure channel. Delete local copies, including any Draft, when they are no longer required.
